To make a payment with PayPal or credit card, please use the paypal button below.

Once you have finished your payment, please complete the Sparkle Course Intake form online.
(Please note this form contains 85 questions and may take 10-15 minutes to complete.)

If you prefer you can download the intake form as a pdf (160kb), or word document (102kb), complete it later then email or post it to us.
